Waiver & Community-Based Services

How This Service Works

Depending on the applicable Virginia Medicaid waiver, payer contract, agency enrollment, and individual authorization, community-based services can include personal assistance, respite, companion services, in-home supports, nursing, day supports, residential supports, and other approved services.

Every waiver service is tied to eligibility, assessment, authorization, and a person-centered plan. The exact services a person can receive are determined by the program and the individual’s assessed needs—not simply by a request for a service.

Amazing Health Care Services LLC should only provide waiver services for which it is properly licensed, enrolled, credentialed, and authorized. Families can contact the office to ask which programs and services are currently available.

Common Components

  • Person-centered planning
  • Authorized personal assistance
  • Respite and companion services

Planning, Documentation, and Coordination

Services should follow the applicable medical order, authorization, person-centered plan, plan of care, or service agreement. Staff document the support provided, communicate significant changes, and remain within the scope of the role for which they are trained and authorized.
