Host Home / Residential Support

How This Service Works

Depending on the program and jurisdiction, residential support may include personal assistance, skill building, supervision, household routines, community participation, health and safety support, transportation, and coordination with the individual’s authorized care team.

Residential services require clear person-centered planning, appropriate provider qualifications, and compliance with licensing, Medicaid, and home- and community-based settings requirements that apply to the specific service model.

Because terminology and program rules vary, Amazing Health Care Services LLC should list Host Home, Sponsored Residential, or another residential service name only when the agency is specifically approved to provide that service.

Common Components

  • Residential supervision and support
  • Daily-living assistance
  • Skill development

Planning, Documentation, and Coordination

Services should follow the applicable medical order, authorization, person-centered plan, plan of care, or service agreement. Staff document the support provided, communicate significant changes, and remain within the scope of the role for which they are trained and authorized.
