How This Service Works
Support may include assistance with personal routines, household responsibilities, health and safety practices, skill development, community participation, appointments, and other activities identified in the person-centered plan.
When transportation is part of the authorized service, staff may support access to medical appointments, waiver services, community activities, employment-related activities, shopping, and other approved destinations.
Supported Living is a regulated service. Availability depends on program requirements, residential licensure where applicable, staff qualifications, authorization, and the agency’s enrollment to provide the service.
