In-Home Supports

How This Service Works

Services may include skill building, personal assistance, household routines, community access, health and safety support, communication, and other person-centered goals identified in the plan.

Staff should work with the individual rather than simply doing everything for them. When appropriate, support focuses on teaching, prompting, practicing, and gradually increasing independence.

The exact activities, staffing level, schedule, and documentation requirements depend on the applicable program and authorized plan.

Common Components

Planning, Documentation, and Coordination

Services should follow the applicable medical order, authorization, person-centered plan, plan of care, or service agreement. Staff document the support provided, communicate significant changes, and remain within the scope of the role for which they are trained and authorized.
